Oo! oo! I know this...

He violated 1) Always keep the gun pointed in a safe direction
and 3) Don't load the gun until your ready to shoot (though I supposed as a police officer, they have to have it loaded all the time...) didn't sound like his finger was on the trigger, so i guess he didn't violate 2...

I'm almost certain he was violating #2. Most of the police .40 S&W guns are Glocks. And those things just never fire due to mechanical failures. They're amazingly well engineered. If it fired, someone or something was pushing on the trigger.

Sounded to me like he had a dud round and was going to clear it when it went off, but hard to say.

Looks like they've changed a few things since I bought mine, but it's basically the same.
